City of Chicago Wellness Program
Chicago Lives Healthy applies to the following CCC employees enrolled in the City Colleges of Chicago Health Insurance Plan as of January 1, 2014:
• All full-time and part-time employees of Local 1600 (except security guards) and spouses/domestic partners/civil union spouses
• Non-negotiable for employees and spouses/domestic partners/civil union spouses
Chicago Lives Healthy is a long-term city wellness Program that uses a powerful yet flexible approach to help improve your health and well-being.

Chicago Lives Healthy is a city wellness program designed to help improve the health and well-being of participants. The dates and information on this page apply to all eligible CCC participants.

The
Chicago Lives Healthy program offers the following services at no cost to you:

• Ongoing health activities, programs, and classes throughout the year
Participate
Join
Chicago Lives Healthy Enrollment is not mandatory for eligible CCC employees
listed above. However, for each employee/spouse/same-sex partner/civil union spouse failing to complete the program requirements by September 30, their employer will increase their monthly healthcare contribution by $50. Your employer will deduct this increase from your paycheck beginning in the fourth quarter of 2014. You can avoid the ongoing deduction of $50 or $100
per month by completing any outstanding program requirements before your next
monthly deduction.
Privacy Protection
Personal
health information is important and must be kept confidential. That's why City Colleges of Chicago and our City wellness programs partner, Health Ways, have strict
policies in place to protect your privacy rights.
All
personal health information shared through the Chicago Wellness
Program will be kept confidential in full compliance with the Health Insurance
Portability and Accountability Act of 1996 (HIPAA) and will only be used and
disclosed as permitted by law.
Registration Steps
To participate in Chicago Lives Healthy, CCC-eligible employees must complete the following steps during the time frame below. Please note that CCC participants participate in a different time frame than other city organizations involved in Chicago Lives Healthy.
Step 1: Schedule and participate in your annual biometric testing
All testing must be completed by May 17th.
A self-scheduler for on-site testing is available from April 10th to May 16th.
Biometric testing is a brief, confidential finger-prick test that measures height, weight, waist circumference, cholesterol, triglycerides, and blood sugar. From May 5th to May 17th, 2014, City Colleges of Chicago will perform biometric testing at their campuses and district offices. Employees and their eligible spouses/same-sex partners/civil union spouses can be tested at available CCC locations.
This is an annual event. If you took a biometric test last year, you will still need to take it again in 2014.

The
self-scheduler will open on April 10th. To schedule your test, please visit the
Scheduling Portal via the link above. Your User ID should be the first letter
of your first and last name, with no spaces. Then enter your date of birth and
the group code that appears under the "Group Code" box. Finally,
click "Log In."
Your family doctor's office also offers in-person biometric testing. Your current health plan benefits include biometric testing done at your family doctor’s office. Biometric test results done by your family doctor must be submitted on a Personal Physician Form. If your family doctor submits your biometric test results on any other form, we will reject them, leaving enrollment step 1 incomplete. After submission, be sure to get a copy of the completed form signed by your doctor and a faxed confirmation for your records.
To get a Personal Physician Form, click on the red button above, enter your first name, last name, date of birth, sex, and zip code, print the Personal Physician Form, and bring it to your testing appointment at your family doctor’s office. You must download your form and cannot use a form with someone else’s name on it.
Step
2: Complete your Well-Being Assessment
Complete your Well-Being Assessment by June 30th.
The
Well-Being Assessment (WBA) is a confidential questionnaire that assesses the
life and environmental factors that are important to your health, well-being,
and your ability to improve both. Please complete this online health survey by
June 30th.
Wellbeing Assessment FAQ
To
take the WBA, please visit the Chicago Lives Healthy Participant Portal via the
link above. If you participated in Chicago Lives Healthy last year, you can
sign in with your username and password. If you are a new participant, please
click on the orange "Sign Up" button and follow the registration
instructions. You will then be able to take the WBA.
Health Check-In Call
After completing your biometric screening and well-being assessment, you will be invited to participate in a check-in call with a certified medical professional. The purpose of the 15-minute call is to provide all Chicago Lives Healthy Wellness participants with the opportunity to benefit from individualized counseling on health and wellbeing.
Completing the call will earn you 6 participation points toward your quarterly goal.
Step
3: Quarterly Health Activities starting July 2014
Starting July 2014, all participants in the Chicago Lives Healthy program will be required to earn 12 points per quarter by completing health program activities.
• 3rd Quarter: July 1, 2014 - September 30, 2014
• 4th Quarter: October 1, 2014 - December 31, 2014

Please note: By the Health Insurance Portability and Accountability Act of 1996 (HIPAA), Healthways protects the confidentiality of your information. Healthways may give employers aggregate data from its workforce, but it does not disclose any individual's health information.
